Welcome Back!

The new school year is quickly approaching! We look forward to seeing the students back on August 14, 2024. Here is a checklist to help get students ready. If you would like paper copies or need assistance please contact the HS Office at 402-497-3501.

The online application for Free & Reduced Lunches is not available at this time. Please download, print, fill out and sign the forms below. We will send out notification once the online version is available.

Athletes are not allowed to participate in any sports practices until all required sports forms are completed.

All Students:

Please complete all required forms.

  1. Back-To-School Enrollment Forms (Required) Completely Filled Out. Log in and select Register for 2024-2025 and follow instructions. Please contact the office if you need assistance.

  2. Student Handbook - (Required) Download, Print and Return Signed Signature Page.

  3. KPCS Activity Handbook (Required)Download, Print and return the signed Signature Page.

  4. Student Photo Release Form (Required) Please Download, Print, Sign and Return.

  5. Health Questionnaire (Required) Please Download, Print, Sign and Return

  6. Request to provide medication - Tylenol, Ibuprofen and Tums (Required) - Download Print and Sign

  7. Prescription Medication Administration Form (If Applicable, Please review and sign to give permission for KPCS trained staff to administer prescription meds)

  8. Asthma Intake Form (If Applicable) Download, Print and Sign

  9. Technology Usage Agreement Form Download, Print and Sign

  10. Student Mileage Reimbursement Form - for K-8 students who travel more than 3 miles to school.

  11. All students that participate in Physical Education must have clean gym shoes with non-marking soles.
